    Beech Brook, one of Northeast Ohio’s premier behavioral health agencies, is currently hiring a Care Coordinator/Case Manager Therapist to provide behavioral health, mental health interventions and perform diagnostic assessment to at-risk youth and families with emotional, psychological, therapeutic interventions in a home based and/or community setting behavioral and/or trauma based issues. The individual selected for this position will be working with children with severe emotional disturbance and their families. Experience conducting assessments and delivering behavioral health counseling in the community is required. The candidate selected for this position must have the ability to support clients with behavioral skills development. This position is a community-based role that requires the therapist to go into homes anywhere in Cuyahoga County to perform interventions. A Bachelor's Degree is required, LSW or LPC is preferred. A minimum of 2-3 years of experience with community-based case management is required.

    Essential Responsibilities (others may be assigned):
    · Provide short term intensive behavioral and mental health intervention in the home in order to
    stabilize families and ensure permanency
    · Partner with interdisciplinary team to coordinate services for families including but not limited to,
    local juvenile court system, psychiatric hospitals, external child serving agencies and child welfare systems.

    Assume responsibility for facilitating the wrap around process and developing a team for each client Attend court sessions, SAR, staffing and other community meetings as necessary for every case Work with family to increase informal support, participation and representation on plan of care and wrap process Conduct a thorough psychiatric diagnostic evaluations, including diagnoses, treatment planning and linkage, as needed to appropriate internal and external services Complete daily progress notes and other Medicaid documentation within proper timeframes Develop client focused treatment plans and monitor for progress Partner with both internal and external treatment providers in implementing and evaluating the ITP/treatment plan

    Develop effective relationships in order to function as a member of an interdisciplinary team
    Ability to work effectively with diverse populations with complex psychiatric needs
    Compliant with contractual requirements and able to complete required Medicaid documentation in mandated timeframe Implement 90-day treatment model with assigned clients

    Education:
    Bachelor’s Degree- Required
    LSW/LPC or higher

    Qualifications:
    · 2 years of experience working with children and families
    · Knowledge of community resources and comfortability in the home setting
    · Must have a thorough understanding of and value a client/family-centered, integrated care
    approach
    · Skill in using computer software applications such as Microsoft Office and electronic medical
    records
    · Documentation such as progress notes (i.e. Psychiatric Diagnostic Evaluations)
    · Knowledge of community resources
    · Experience working with diverse populations with complex needs, including high risk clients
    · Ability to work a rigorous schedule to handle crisis situations
    · Excellent communication and relationship building skills
    · Strong organizational and time management skills
    · Flexible and adaptable in order to meet the needs of the family

    Preferred Qualifications:
    · Experience working with clients in inner city homes
    · Experience working with medical records
