The In-Home Mental Health Counselor is responsible for coordinating and providing behavioral and mental health service and support to their assigned clients and families or care givers. They are required to provide direct service, complete required paperwork and Mental Health Assessments according to the Ohio Department of Mental Health guidelines, as well as maintaining ongoing contact and linkage to services with other community resources as necessary.
Essential Functions:

Develops a therapeutic relationship with each client. Provides individual, group or family therapy as needed. Available for clinical consultation.

Therapist maintains a minimum productivity level of 52% billable face-to-face contacts per week. Part-time therapist (up to 29hrs/wk) maintains minimum productivity level of 52% billable face-to-face contacts per week.

Maintains ongoing frequent contact with other assigned OhioGuidestone professionals, parents or outside agency professionals assigned to case. Provide CPST and minor case management functions if needed.

Assumes a strong advocacy role for clients and makes recommendations for clinical and support services needed upon discharge.

Actively participates in all treatment conferences and meetings for the clients.

Completes all paperwork required by the agency in a timely manner.

Assumes a leadership role in the interpretation and support of agency policies.

Participates in agency and community meetings as needed. Attends conferences and workshops to maintain professional competence. Provides in-service training as requested.

May be involved in the planning, organization, and implementation of special events or department specific projects or seasonal program changes.

Organization and maintenance of client case files including: client information and case notes.

Consult with school personnel and participate in team meetings as necessary.

Meet with school personnel to establish parameters of school based behavioral and mental health services and meet with school personnel on an as needed bases or as requested throughout the school year.

Inform appropriate school personnel of clients being served maintaining confidentiality.

Educate school personnel on various mental health conditions/diagnoses and assist with effective classroom intervention/strategies

Timely completion of Mental Health Assessments, ISP’s and Terminations.

Recordkeeping and copying of necessary information, dissemination to appropriate individuals within the agency and any outside agencies based on departmental requirements.

Participate in staff training, consultations, supervisory and staff meetings.

May be involved in the planning, organization, and implementation of special events or department specific projects or seasonal program changes.

In times of need, the counselor may be asked to complete related duties other than those indicated above as assigned by their supervisor/ department.
If Counselor is also responsible for School Services, the following functions are essential, in addition to above:

The Counselor is responsible for coordinating and providing service and support to their assigned clients and families or care givers in the school environment.

When applicable, provide school personnel and parents with information on child’s mental health diagnosis, behavior management, household management, communication and relationship enhancement.

Provide group services, leads treatment and psycho-educational groups.
Performance/Physical Requirements:
-
Work is performed in the home/community/school setting of clients. This could involve extensive driving.

English reading and writing skills required

Interacts verbally with coworkers and representative from outside agencies, school personnel, & families/caregivers.

Attention to detail and confidentiality required.

Works flexible hours which will involve nights and/or weekends and required to participate in on-call pager system.

Requires typing and computer competency, including Windows applications for moderate levels of typing (case notes and other paper work).

Ability to manage own time and schedule.
Qualifications:
-
Master’s Degree in Social Work or Counseling required
-
Ohio LPC/LPCC/LSW/LISW required
-
Must have a valid Ohio Driver’s License and safe driving record
