Professional Counselor with Ohio license needed to join a Federally Qualified Health Center with office locations in Lisbon, East Liverpool, and Salineville. 

Counselor will:

·         Integrate Behavioral Health Services in the Primary Care setting.

·         Working closely with medical providers to immediately address need areas identified during medical appointments.

·         Provide diagnosis and counseling in accordance with established health center protocols and within the provider’s scope of practice.

·         Coordinate services with medical providers on staff as well as in the community.

·         Prepare a detailed specific services/treatment plan on each participant on his/her case load;

·          Job Requirements:

• ·         Masters in counseling required
• ·         Licensed to practice in the state of Ohio
• ·         1-2 years experience preferred
• ·         Rotating On Call
