    The Social Worker/Counselor will be responsible for the treatment coordination of behavioral health and substance abuse services for each adult and youth customers.

    Duties include: complete intake forms and assessment in order to determine appropriate services and/or treatment; assist clients with establishing goals and developing a treatment plan/service plan; reviewing client's needs and progress, and updating treatment /service plan as needed; provide individual intervention as needed to work toward goals established in the treatment/service plan; complete required paperwork/documentation of services in a timely and accurate manner; provide supervision as directed by the Health & Wellness Clinical Supervisor and/or Operations Director.

    Qualifications include: Bachelor's Degree and Ohio LSW credential required (Mater's Degree and Ohio MSW/LPC/PCC credential and one-year experience in alcohol, substance abuse or behavioral health field preferred); initiative to develop services and work with professionals in both the public and private sector; excellent verbal and written communication skills including the ability to effectively communicate with individuals and large groups; operational knowledge of PC Operating Systems and Microsoft Office software package; valid driver's license, automobile insurance and reliable transportation; ability to pass an Ohio BCI & FBI background check and to satisfactorily complete pre-employment drug/alcohol testing.

    Job Type: Full-time

    Pay: $21.63 per hour with benefits
